- Accomplished Application Form
- Report Card of the previous school year
- Certificate of Awards
- Photocopy of PSA Birth Certificate
- Photocopy of ITR and current payslip
- Photocopy of current electric bill
- 2 Recommendation Forms from the Principal, Class Adviser or Guidance Counselor ( Grade 7, 8, and 11)
- ESC/QVR Certificate for Grades 8 & 11 applicants
